When you submit a form, we store your submission as a lead record tagged with your topic, where you came from, and any referral code. Dr. Marla can see these lead records and summary reports in a private Admin Area that is not visible to the public.

This website does not create a provider relationship or medical record system. If CoachSmart Health later adds clinical services, patient portals, telehealth, insurance billing, or protected health workflows, this Privacy Policy should be reviewed before those services go live.
